Support Plans

If you’re a woman caring for a person with intellectual disabilities, you’re my hero. Mothers, sisters, friends and direct-care professionals, that means you.

I want to give something back to you; you who have given so much. As such, I’m glad to offer motivational coaching and support planning for you.

Who would benefit from this unique experience? Consider giving it a go if…

  • You’re feeling overwhelmed. You know that there has to be a way to get your needs met while meeting the needs of others, but it’s not happening.
  • You have dreams and goals beyond care-giving, but you aren’t sure how to accomplish them within the context of your current life.
  • You’ve resonated with the content at A Wish Come Clear and the Creed Of Care, but find yourself wishing you could connect on a more personal level. Despite your best intentions, you’re struggling to incorporate good self-care into your daily life, and you’d like someone to talk you through the process.

I’d imagine you’re asking:  What can I expect to get out of a session like this? What value will it bring to my life?

  • You’ll receive compassionate, attentive listening. You and your story will be the primary focus. I’m here for you. This is your time, and we will focus on how to help you get what you need from it!
  • You’ll have a chance to explore the way care-giving affects all areas of your life. We’ll cover topics such as:  home life, time management, health, work, communication and relational expectations.
  • You’ll receive a tailored-to-you support plan and goal-tracking tool. We’ll create these together, and I’ll send them to you after our session. The templates are based upon something called an ISP (Individual Support Plan) that’s required annually for people with intellectual disabilities receiving Medicaid Waiver services. I’ve maintained the spirit of the plan (ensuring that a person sets goals both important to and important for them), eliminated the bureaucratic hassles and made it more fun!

In case you’re wondering what skills and abilities I bring to the table, I bring…

  • Over 4 years of professional direct-care-support experience, working in several capacities:  as a direct-care assistant; as a home life coordinator (accompanying a team of direct-care assistants, providing counsel and practical supports); as a program director (overseeing the care of eight individuals with intellectual disabilities; providing training for new assistants; speaking as an advocate for people with disabilities).
  • As program director for L’Arche, I’ve written dozens of Individual Support Plans, helping people achieve their goals.
  • Training in Positive Behavioral Supports; author of multiple Informal Behavioral Support plans. Also trained in Non-Violent Communication.

Note:  these are special introductory rates, effective for sessions scheduled before July 1, 2011. (Get them while they’re hot!)

  • Support Package #1 includes:  A 1-hour session via Skype, and the personalized support plan and tracking tool. $125.
  • Support Package #2 includes: A 1-hour Skype session and the personalized support plan and tracking tool with personalized follow-up. I’ll touch base with you weekly via email in the month-long period following our session to support you in your specific self-care game plan, AND we’ll do a 30-minute Skype session at the end of the month to follow up further. $250
  • Support Package #3 includes:  An initial 1-hour Skype session and the personalized support plan and tracking tool with lots of personalized follow-up. This package includes monthly 30-minute Skype sessions in the 12 months following our initial session. These monthly check-ins will give you a high level of support as you implement your specific self-care game plan. I’ll also incorporate quarterly revisions of the support plan and tracking tool at your request. $900.
